ABSTRACT

Revista Médica de Chile was founded in 1872 by Sociedad Médica de Santiago (currently Chilean Society of Internal Medicine) and it is one out of twenty five medical journals founded during the XIX Century that still remain active. In monthly issues it presents clinical and biomedical research articles, as well as research and position papers dealing with medical education, public health or medical ethics. Special attention receive the impact on health care given by the emergence of new private medical schools in Chile, the relevance of methodologies to assess clinical competences in graduates, the certification of specialty programs and specialists, the appropiate use of simulators in clinical training, telemedicine, organ transplants, and current issues in medical ethics, such as abortions and euthanasia. Institutional and international collaboration in medical research is clearly reflected in recent years. Articles are subjected to external peer review and ICMJE Recommendations are carefully considered throughout the review process. Most articles are published in Spanish but English abstracts are mandatory and a growing number of articles are currently published in English. An Open Access electronic version is published by SciELO. Revista Médica de Chile is indexed in the most important international data bases and it is a member of the ICMJE ruling committee. The long life and prestige of this journal can be considered a testimony of the cultural heritage and progress in Chilean medicine.

ABSTRACT

En este artículo se profundizó sobre la vida del médico Gerardo Ignacio Acosta Peñalver que laboró en el poblado de San José de los Ramos y en el Hospital Dr. Mario Muñoz Monroy de Colón, con el objetivo de destacar su entrega y consagración a la medicina; y de esta manera, rendirle un merecido homenaje. Se abordaron aspectos de su vida y obra; se destacó su superación constante principalmente en la especialidad de Medicina interna y el ejemplo inolvidable en sus familiares (AU).


The life of the doctor Gerardo Ignacio Acosta Peñalver Hospital was treated in this article. He worked in the village of San Jose de los Ramos and in the Hospital ¨Mario Muñoz Monroy¨ of Colon. The aim was highlighting his devotion and consecration to medicine, and that way rendering him a well-deserved homage. Several aspects of his life and work were approached: his constant upgrading mainly in the specialty of Internal Medicine and his unforgettable example to his relatives (AU).

ABSTRACT

The authors review the most important contributions of Pierre Marie to the elucidation and description of several neurological diseases, such as Charcot-Marie-Tooth’s disease and hereditary cerebellar ataxia, as well as his contributions to Internal Medicine, including his pioneering studies on acromegaly, ankylosing spondylitis, and hypertrophic pulmonary osteoarthropathy. His works led to incontestable advances in the medical sciences that transcended his time.


Os autores revisaram as mais importantes contribuições de Pierre Marie para a elucidação e a descrição de várias doenças neurológicas, tais como a doença de Charcot-Marie-Tooth e a ataxia cerebelar hereditária, bem como na medicina interna, incluindo os seus estudos pioneiros na acromegalia, espondilite anquilosante e osteo-artropatia hipertrófica pulmonar. Os seus trabalhos promoveram uma crescimento incontestável das ciências médicas que transcenderam o seu tempo.

El Colegio Médico de Cundinamarca es la institución gremial médica más antigua de Colombia con 98 años de existencia. De su seno surgió la Federación Médica Colombiana, que como entidad de orden superior, agrupa a los Colegios Médicos Departamentales, entidad que registró hace unos meses la celebración de su cumpleaños número 78.Estamos aquí reunidos, en el Auditorio Guillermo Fergusson Manrique del Hospital de San José en sesión solemne, para imponer la máxima condecoración que otorga nuestro Colegio a un distinguido colega, cuyo ejercicio profesional ha sido digno, honesto, ético y al servicio de los intereses de la salud de los colombianos.En estos tiempos turbulentos, en donde el capital financiero arremete contra todas las instituciones de la salud y la seguridad social que en su desarrollo histórico había logrado Colombia, la actitud honorable y enhiesta de los condecorados por el Colegio Médico de Cundinamarca y Bogotá sirve de ejemplo para todas las generaciones de colegas y de profesionales del sector de la salud. Y ese es uno de los motivos por los cuales honramos al Profesor Hernández hoy día. También resaltamos en él la dedicación a su propia causa, que ha sido inspiradora para quienes hemos compartido en su grata compañía trayectos de la vida estudiantil, laboral, gremial o las luchas por un sistema de salud más justo para los colombianos.Honrando el nombre de la condecoración y para quienes no lo conocen, evoco la memoria del memorable científico César Uribe Piedrahita, nacido en Medellín en 1896. Estudió medicina en la Universidad de Antioquia y se especializó en la Universidad de Harvard, en 1921. Estando en los Estados Unidos de Norteamérica publicó algunos trabajos sobre protozoarios y parasitología en revistas especializadas.

ABSTRACT

Existe una antigua fábula popularizada en un poema escrito por el poema norteamericano John G Saxe en 1860. En él se demuestra como podemos estar equivocados si sustentamos nuestras opiniones con base a una insuficiente evidencia obtenida a través de un inadecuado estudio. Cada uno de los seis ciegos del poema se acercó al elefante para investigar cómo era pero tomando solo una parte aislada del animal. Cada uno se hizo de una solida pero errónea opinión de lo que realmente era un elefante. Discutieron entre sí defendiendo sus impresiones y como Saxe escribiera, "Los ciegos disputan y se querellan: cada uno está seguro de haber hecho bien su prueba...¡Cada uno tiene un poco de razón...y todos están equivocados!". El autor asimila al elefante a un paciente cuya queja es interpretada de manera diferente al ser analizada a través del juicio prejuiciado de diversos especialistas al interpretar fragmentos inconexos del todo indivisible que es el ser humano


There is old Indian fable made popular in a poem written by John G. Saxe in 1860. It demonstrates how we can be so very wrong by basing our opinions on insufficient evidence gained through inadequate studies. Each of the six blind men in the poem walked up to an imposing elephant to investigate what it investigate what it was-but each touched only one part of the animal. Each man had a faulty yet strong opinion of what an elephant was really like and disputed the others with great vigor. As Saxe wrote, "through each was partly in the right, and all were in the wrong! "The author compares the elephant to a patient whose complaint is interpreted differently when analyzed through the prejudiced judgment of various specialist who interpret unconnected fragments of the fully indivisible human being

ABSTRACT

No presente trabalho foi realizada uma reflexão sobre a condução do ensino médico no Brasil. Afirma-se que os catedráticos de Clínica Médica da Faculdade de Medicina do Rio de Janeiro, e em especial o Professor Clementino Fraga Filho, tiveram papel de liderança neste processo. A tese pretendeu mostrar que, acima de quaisquer modelos apontados pela bibliografia especializada – franceses, germânicos ou norte-americanos - o grupo de catedráticos clínicos que chamamos de “geração dos anos 1950” acreditou que a Clínica Médica seria a base do ensino de medicina. Apresenta-se, aqui, então, a 1ª Cátedra de Clínica Médica, do Professor Clementino Fraga Filho, como um arquétipo de excelência de ensino. Além de terem buscado estimular a homogeneização dos currículos das diversas faculdades de medicina do país, estes professores propuseram a criação de departamentos, em oposição à instituição tradicional “cátedra”, que, por ser conduzida por um único indivíduo, seu ‘proprietário’, constituía-se um entrave para a qualidade do ensino da medicina. Os catedráticos clínicos elaboraram propostas de uma reforma para o ensino médico e para o ensino superior, em geral, que culminaria, em muitos pontos, num evento distinto de suas perspectivas iniciais, a Reforma Universitária de 1968. Suas aspirações curriculares e estruturais para o ensino médico foram frustradas, pois, longe de resultar em maior diálogo entre as diversas disciplinas, a Reforma terminaria em sua fragmentação.


This work is a reflection about the leading of medical teaching in Brazil. We assert that the Professors of Internal Medicine of the Medical School of Rio de Janeiro, specially, Professor Clementino Fraga Filho, had had a principal role in its process. Our thesis intends to show that, overall any “educational model” - as a French, or a German or an American one – that is pointed out by a specialized bibliography, there is a group of ClinicalProfessors that we call here “1950’s generation” who believed that Clinical Medicine served as the main basis for medical teaching. We present the 1st Chair of Medical Clinics as a locus of expertise in Brazilian medical education. Besides they had given a raise to themedical curricula trying to get them more homogenous, in many Brazil Medical Schools, the clinical Professors suggested the creation of departments, in opposition to the traditional Chairs, because they believed that the Chairs were a problem to medical teaching. These Professors believed that the Chairs, leaded by only one Professor, so called “the owner”, should get worse the medical schools performance. The clinical Professors had created many propositions to a great medical teaching reform as well to the University teaching. These propositions would reach, in different aspects, a very different happening from their purposes – the University Reform of 1968. The Professors’ curricular and structuralaspirations to the medical education have been frustrated because the Reform didn’t result in a better dialog among different disciplines, but in its fragmentation.

ABSTRACT

Trata-se de um depoimento do médico Clementino Fraga Filho. A trajetória deste reverenciado professor de clínica médica da UFRJ, diretor da Faculdade de Medicina e reitor da mesma instituição - que assumiu com desenvoltura a construção do Hospital Universitário, o qual leva, em justa homenagem, o seu nome - esclarece e informa alguns dos fatos que marcaram definitivemente a educação médica na segunda metade do século XX. A paixão e as idéias de um homem que foi uma liderança acadêmica num contexto de profundas mudanças no ambiente da formação e do exercício da medicina transparece, aqui, numa narrativa saborosa.
